Life Care Planning
Informed choices for our loved ones
As they age, many people experience a surge of
anxiety based on several factors:
- Declining health and function
- Increasing loneliness or isolation
- Helplessness
- Loss of independence
- Fear of losing their assets to long-term health care needs
This anxiety can lead to an overwhelming fear
of dependence and loss of purpose.
The children of aging parents also experience a
surge of anxiety from:
- Financial hardships associated with the high
costs of care
- Physical exhaustion
- Increased responsibility for making decisions about their loved one’s care
- Anger and guilt about not being able to do
more
- Loss of personal freedom
- Fear of loss
- Feelings of inadequacy in assessing their loved
one’s needs and options
Aging doesn’t have to bring hopelessness or despair! We can help!
Through the Life Care Planning process, we bring
experience and resources to alleviate your concerns
and provide answers to your questions about the
issues facing you and your loved ones.
A successful Life Care Plan involves the whole family.
Together, we develop a Life Care Plan that is customized
to fit your family’s specific needs and desires.
Our goals are:
- To ensure that you or your loved one gets good care whether at home or in a residential facility.
- To help you protect and preserve the assets you
have accumulated by considering the options
available to you to cover the cost of care.
- To help you and your family make the sometimes
difficult decisions relating to your long-term needs.
To prepare a Life Care Plan that is right for you, we will:
- Evaluate your physical and mental abilities to
determine which of the available options will best
provide for your safety and well-being.
- Assess your ability to manage your everyday life
and, if needed, refer you to services which may
enable you to continue to live independently.
- Design an Asset Protection Plan that includes
the preparation or revision of estate planning
documents, asset transfers, applications, etc.
- Develop an on-going plan to monitor your
care to ensure the highest possible quality of life.
A Life Care Plan includes all necessary legal documents,
consultations, referrals, and other services to
provide you and your family with complete peace of
mind.
